if you need to create a pcb and your gerber are ready to go, you can done that through www.pcbcart.com.
They are located in china and the price are very good. Ie, My company located in europe have already done many pcb  (generally 12 layers with bga mounting) and the result is very good. don't hesite to go there, even for a one layer proto pcb. the website is easy to use, you can easy create a online quote and pay with credit card.
In one word, perfect!

I have just quoted 200 boards (100x50) mm for under 100$ total (shipping not included) 
Those are quoted for 287$ in my country …. Just how they can do that ?

1) labor cost is about 1/7  comparing to western country.
2) PCB fab is high pollution industry, high cost for western country in terms of environment protection compliance. there are minimum cost to meet the requirements.
3) competition is high in that region.  as long as they can pay for the employee salary, they are willing to do it with lower profit margin. (to keep business going)
4) volume quantity lowers the cost,  you share the NRE cost with others, when they make the PCB they put several different project in one PCB run.
5) You need to add the shipping cost to get the real TOTAL price !  the international shipping  cost is very high now.( high oil price)
6) For prototype,(5-10pcs).  the cost will be about the same.  (RMB$300)+shipping
