Which opamp meets the following :-

Supply Voltage : +/-18V or +/-15V rail to rail.
Bnadwidth: >= 50 Mhz at gain=10
Slew rate: >= 1000uV/s

maybe if you said what it's for you might get an answer but I don't think your going to find one exactly like that with rail to rail with that voltage,there's a few but the majority are 5volt or less
heres one that is as close as i could find

it is for max038 based signal generator. so it must be of high slew rate (max038 output 20MHz).

Try LM6181, LM7171, THS6052, THS6053 and generally op amps used as ADSL line drivers.
